Pre-Chemo MRI Could Spot Breast Cancers That Will Still Return
Some breast cancer patients who seem cured still relapse — this MRI might catch them early.
Some breast cancer patients are told the tumor completely disappeared after chemotherapy — it's called a pathological complete response, or pCR, meaning no cancer was found during follow-up surgery. That sounds like a cure, but it isn't always. In 5–15% of these 'perfect responders,' the cancer eventually returns, and doctors haven't had a reliable way to know who is at risk.
This study, which analyzed more than 1,200 patients across four independent groups, suggests the answer might be hiding in the MRI scan taken before chemo even begins. Using a special type of MRI called DCE-MRI (which tracks how dye flows through the tumor), researchers measured how visually uneven the tumor was. They used computer analysis to calculate an 'entropy' score — basically a measure of how messy or chaotic the enhancement pattern looked. High entropy meant an adverse structure; low entropy meant a favorable one.
The results were striking. In one group of HER2-positive patients who had no cancer remaining after chemo, only 12.5% of those with a favorable pre-treatment pattern relapsed within years, compared to a shocking 80% of those with an adverse pattern. The MRI didn't replace the pathology test — it added a new layer of information. Among patients with a complete response, about a quarter still showed an adverse structure, and those people had roughly 2.9 times higher risk of recurrence.
Why would the tumor's MRI pattern matter before treatment? The researchers found it connects to the immune system. Favorable tumors appeared to have active immune responses, while adverse ones looked immune-depleted — like the tumor had already found a way to hide from the body's defense. So the scan may act as a cheap, non-invasive window into the tumor's biology. It's not ready for clinics yet; prospective trials are needed. But for patients, this could eventually mean more personalized after-chemo care — not just breathing a sigh of relief, but knowing if you need extra monitoring or prevention.
